10+ years clinical work with gay men

The point this is for

You may already understand yourself. That has not changed the pattern.

A lot of gay men arrive at a strange place in the work. They can explain their childhood. They can name attachment styles. They can talk about shame, avoidance, people-pleasing, desire, and old wounds with real intelligence.

Then something happens.

Someone gets close.
Someone pulls away.
A message goes unanswered.
A body does not look right in the mirror.
A relationship starts feeling real.
A room feels unsafe before anyone has said a word.

The old pattern takes over before the adult self has finished reading the situation. That is the point this programme is built for. Not the absence of insight. The gap between understanding the pattern and being free of it.

Who this is for

You may recognise the pattern.

This is for gay men who are psychologically literate, outwardly functional, and privately stuck. Not in crisis. Not looking for a course or a pep talk.

If most of these land, the work is likely worth the conversation.

  • You lose interest when someone is finally consistent, and chase the men who cannot meet you.
  • You can explain your childhood clearly, then become twelve years old the moment someone pulls away.
  • You use sex to feel chosen, then feel emptier afterwards.
  • You know the body shame is irrational and still manage your appearance like a second job.
  • You become charming, competent, useful, or detached before anyone gets close enough to see what you need.
  • You have done enough therapy to understand the story. The pattern just keeps finding new outfits.
  • You keep saying you are fine because needing more feels embarrassing.
  • You do not want vague reassurance. You want the pattern named properly.

Midlife man sitting alone in a quiet living room, holding a mug and looking thoughtfully toward the window.

This work is for the man who looks functional from the outside and still knows something keeps repeating underneath. He may have language. He may have insight. He may have done therapy. The pattern still has a body.

What you leave with

The sessions are the process.
The Pattern Map is what you keep.

At the end of the six sessions you receive a five to seven page written document naming the main patterns we identified. Where each likely began, how it operates now, what it protects you from feeling, what it costs, and what to watch for next.

It is written in plain language and built to be returned to long after the sessions end. The point is not a beautiful document. The point is to make the pattern harder to obey blindly.

  • Six 60–90 minute private sessionsOnline video, one-to-one with Gino Cosme.
  • Between-session reflection materialA specific frame or question after each session to develop the work.
  • Written Pattern Map at completionFive to seven pages. Plain language. Yours to keep.
  • What it is notNot a diagnosis. Not a clinical report. Not a worksheet. Not AI-generated.
What this is

A psychological mapping intensive for gay men.

The Formation Programme is structured one-to-one work focused on how your patterns were formed, what they protect, how they operate now, and what becomes possible once they are named clearly. The focus is not advice or motivation. It is formation.

Many gay men were shaped by more than family. They were shaped by rooms. Who was safe. Who was watching. How much softness was allowed, how much desire had to be hidden, how much need was too much. Over years, those adaptations start to look like personality: independence, charm, control, sexual confidence, emotional distance, hypercompetence. Easy to have around. Hard to actually know. This programme is built to map those adaptations with care and precision.

What we may map together
Relationship repetition Avoidance and pursuit Fear of closeness Attraction to unavailable men Body shame and vigilance Sex as regulation or proof Shame and self-attack Anger under charm and competence Family and father wounds Emotional self-protection Loneliness Over-functioning People-pleasing ‘I’m fine’ as a survival strategy
The six sessions

A bounded arc across formation, shame, relationships, sexuality, identity, and desire.

Six conversations, in a deliberate order. Each session feeds the next. The last one is where the map gets sharp.

Session One
01
Formation
The environments (family, religion, culture, school) that shaped how you learned to manage your identity, your needs, and your visibility.
Session Two
02
Shame Architecture
Where the shame lives, what it protects, and how it shows up in behaviour that does not look like shame from the outside.
Session Three
03
Relational Patterns
Who you choose, how close you get before you pull back, what intimacy costs you, and where avoidance is doing the steering.
Session Four
04
Body & Sexuality
Appearance, performance, what you allow yourself to want. Where the body became a project rather than a place to live.
Session Five
05
Identity vs Performance
What is genuinely yours and what was learned. Who you are when the performance stops, and what it has cost to keep it running.
Session Six
06
What You Actually Want
Not what you think you should want. With a more accurate map, what direction does that suggest, and what becomes available that was not before?
From the clinical work

Three patterns that recur most often across 10+ years.

The programme exists because these keep arriving in the consulting room with different surface details and the same underlying structure.

Pattern 01

Pursuit–withdrawal cycle

The pattern that looks like confidence and operates as avoidance. Intensified interest near closeness, followed by structured retreat the moment reciprocity arrives. Reads to others as composure. Reads inside the body as something else entirely.

Pattern 02

Performance as identity

Competence used as protection. The version of you that earns its place by being useful, articulate, or impressive. What gets sacrificed is the part underneath that wants to be known without performing first.

Pattern 03

Aesthetic vigilance

The body managed as a project, not lived in as a context. Mirror checks, food rules, weight self-surveillance. Imported from shame, not from health.

Scope

This is not therapy. It is not coaching.

The programme is bounded, structured, and focused on naming the psychological patterns underneath everything else. If you need clinical treatment, choose therapy. If you want motivational accountability, choose coaching. This is neither.

How it works

Six steps from first conversation to written map

One private container. Clear edges. No phone tag, no open-ended drift.

  1. Fit consult (€100, 30 min)We look at what brings you here, the pattern you want to understand, and whether the structure fits. If it does not, I will say so.
  2. Intake questionnaireA focused questionnaire identifies the patterns, themes, and relationships we may need to map.
  3. Six private sessionsSix 60 to 90 minute sessions across recurring patterns rather than isolated events.
  4. Integration materialBetween sessions, focused prompts to notice how the pattern operates in ordinary life.
  5. Written Pattern MapA five to seven page written map delivered after the final session.
  6. Next-step guidanceWhere appropriate, we discuss whether continued therapy or other support makes sense afterwards.
Clarity on fit

This is not the right fit if

The work is structured and reflective. It needs you to be stable enough to stay with discomfort. It is not for you if:

  • You are in acute crisis or need urgent mental health support.
  • You are experiencing active suicidal ideation.
  • You need psychiatric care, diagnosis, medication support, or intensive clinical treatment.
  • You want weekly therapy as your main form of care.
  • You are looking for advice, motivation, dating strategy, or someone to tell you what to do.
  • You mainly want validation, without examining your own part in the pattern.

About the clinician

Gino Cosme

Psychotherapist · Writer · Specialist in gay men’s psychology

Gino Cosme is a psychotherapist specialising in gay men’s emotional lives, identity, shame, relationships, and the patterns that form when you spend years adapting to environments that did not fully see you.

The Formation Programme draws on more than ten years of clinical work with gay men across Europe and internationally, around shame, intimacy, emotional self-protection, body vigilance, loneliness, family wounds, avoidance, anger, and the private cost of appearing fine. It distils that work into a structured six-session process for men who want focused psychological clarity outside open-ended therapy.

If it isn’t a fit

You are not locked in.

If after session one you decide the programme is not the right fit, everything beyond that first session is refundable inside a seven day window. The first session is retained at €250, because the work was delivered. No questions, no friction.

The work only lands when both clinician and client feel it is working. If it is not landing for you, neither of us benefits from continuing.

A note on this first public version

No programme testimonials yet. By design.

This is the first public version of The Formation Programme, so there are no programme testimonials yet. The structure is new. The clinical work behind it is not.

It has been built from more than ten years of work with gay men around shame, intimacy, emotional self-protection, body vigilance, loneliness, family wounds, avoidance, anger, and the private cost of appearing fine.

As the first participants finish, I may add feedback here, with their explicit permission and nothing quoted without it. Until then, I would rather be clear than perform credibility I have not earned yet.

Booking

If the pattern keeps repeating, map it properly.

You do not need another promise to become your best self. You need to understand why the same pattern keeps returning, what it protects, what it costs, and what becomes possible once it is finally named clearly. Start with a 30-minute fit consult. If it is not the right work for you, I will tell you.
